3. Pay for ASB Card ($20/year) and Participation Fee ($150/sport) online or by check/cash

BISD athletics has partnered with FinalForms, an online forms and data management service. FinalForms allow you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking that ALL parents or athletes use FinalForms.

Please register at FinalForms and follow the prompts to create your account, create your students and sign your forms. 

All athletes must have a current medical eligibility form on file in the office before turning out for a team. Physical exams are considered current for two years from the date of the exam. This WIAA Form is the preferred physical form, but we also accept physical forms from pediatrician's offices as long as they state that the athlete is cleared to play sports without restriction.

Each Athlete Must Must Pay The Following Fees:

• ASB Card: Online payment for the ASB card  is $20 for the school year
• Participation Fee: $150/sport participation fee

Fees are payable as soon as an athlete makes a roster and prior to the first competition.  These fees may be paid through the online payment site or by check or cash at our attendance office.  

Do you need help paying these fees?

If your family needs assistance with these fees, please contact your child's counselor.  No student will be turned away from participating in any school activity based on their ability to pay.
