• Tryouts will be 1/12, 1/13, 1/14 from 3:45 to 5:15
• Registration is currently open: Link to Registration Page
• Register your student using Final Forms: Link to Final Forms  
• Take physical forms to your doctor after printing out from Final Forms: Preparticipation Physical Eval Form (Stays with Doctor), Physical Exam (Stays with Doctor), and Medical Eligibility Form (Doctor completes and you Upload into Final Forms). 
• All students should have a completed Medical Eligibility form uploaded to Final Forms to try out.
• Once teams are selected, pay for ASB Membership Card ($25/year) and Participation Fee ($150/sport) online or by check/cash at the Woodward Attendance Office.
• Complete the Online signatures on Final Forms for both families and students.
• Practices will run daily Monday through Friday from 3:45 to 5:15 pm.
